Miss L was discharged from hospital to a short term residential placement. She had no idea how she was going to cope with being discharged home especially as she also needed to move to more suitable accommodation.

She was referred to support services who assisted her in arranging to get her new accommodation decorated carpeted and curtained and to terminate the tenancy on the old property and arrange removals from one to the other so that the new property was ready for occupation.

Miss L then wondered how she would cope with day to day tasks such as washing ironing and shopping. Again support services were able to offer regular help with these tasks and call in weekly with shopping and fortnightly with laundry.

Her next hurdle was a doctors appointment. She hadn’t been out of her new flat and now had poor mobility and felt very vulnerable. Support services were able to arrange to collect her and support her at the doctors with her appointment and ensure she understood what the GP had said. They also set up a medication renewal service whereby they knew when medication was due, arranged repeat prescriptions and collected them in time.

Miss L really wanted to regain her independence but had lost confidence with cooking and budgeting so the Support Team were able to help her set up and manage a budget and once a month spent a day with her preparing and cooking simple meals. Miss L gained a great deal of pleasure and confidence from this activity.
